3PB Player Waiver
Toronto August 15, 2026 Tournament

1. Identification of Parties

This Waiver and Release of Liability ("Agreement") is entered into between the participant named above ("Participant") and Three Pillars Foundation, operating as Three Pillars Basketball, including its directors, officers, employees, volunteers, sponsors, venue owners, and affiliates (collectively, "Organizers"). This Agreement applies to participation in all tournament activities, including games, warm-ups, travel within the facility, and any related events.

2. Assumption of Risk

I acknowledge that participation in a basketball tournament involves inherent risks, including but not limited to falls, collisions, physical injury, sprains, fractures, concussion, permanent disability, paralysis, and death. Risks may arise from my actions or those of others, equipment or facility conditions, weather, or the nature of the sport. I voluntarily assume all such risks, whether known or unknown.

3. Release and Waiver of Liability

In consideration for being permitted to participate, I release and discharge the Organizers from all claims, demands, damages, or liabilities arising from participation in the tournament. This release applies to ordinary negligence but does not apply to gross negligence, reckless misconduct, or intentional harm, which cannot be waived under law.

4. Medical Authorization

I authorize licensed medical personnel to provide any treatment deemed necessary due to injury or illness arising from participation. I accept full financial responsibility for all medical expenses incurred.

5. No Insurance Provided

I understand that Three Pillars Foundation / Three Pillars Basketball does not provide medical, accident, or health insurance. It is my responsibility to obtain such coverage if desired.

6. Equipment & Facility Disclaimer

I acknowledge that I am solely responsible for ensuring my personal equipment and footwear are safe and appropriate. I further acknowledge that athletic facilities and equipment may have hazards inherent to sports environments.

7. Personal Property Disclaimer

I acknowledge that Three Pillars Foundation / Three Pillars Basketball is not responsible for any lost, stolen, or damaged personal belongings or equipment brought to the tournament venue. I understand that I am solely responsible for safeguarding my personal property at all times.

8. Communicable Disease & Illness Acknowledgment

I confirm that I am not experiencing symptoms of illness at the time of participating. I understand and assume the risk of exposure to communicable diseases, including but not limited to COVID-19, and release the Organizers from related claims to the extent permitted by law.

9. Indemnification

I agree to indemnify and hold harmless Three Pillars Foundation / Three Pillars Basketball from any claims arising from my actions, conduct, or breach of this Agreement.

10. Compliance with Rules and Code of Conduct

I agree to follow all tournament rules and instructions from officials or staff. I understand that unsportsmanlike conduct, harassment, violence, intoxication, or failure to comply with rules may result in removal without refund.

11. Photo, Video, and Media Release

I grant Three Pillars Foundation / Three Pillars Basketball full permission to capture and use photographs, audio, and video recordings of me for promotional, educational, advertising, and social media purposes. This includes use on websites, newsletters, printed materials, and digital platforms.

I waive all rights to compensation, royalties, or approval of the final product.

If I do not consent, I may request a written opt-out form.

12. Consent to Receive Emails and Text Messages

By signing this Agreement, I consent to receive emails, text messages, and other electronic communications from Three Pillars Foundation / Three Pillars Basketball regarding:
  • Tournament updates and schedules
  • Future events, programs, camps, and announcements
  • Emergency notifications

I understand I may unsubscribe at any time by following the instructions provided or contacting the organizers.

13. Refunds, Weather, and Event Modification

I understand that schedules may change due to weather, safety concerns, or facility issues. Refunds are not guaranteed unless explicitly stated by the Organizers.

14. Transportation Disclaimer

I acknowledge that the Organizers are not responsible for transportation to or from the event.

15. Severability

If any portion of this Agreement is found unenforceable, the remaining provisions shall remain in effect.

16. Governing Law

This Agreement is governed by the laws of the Province of Ontario. Any dispute shall be resolved exclusively within Ontario.

17. Acknowledgment of Understanding

I confirm that I have read and fully understand this Agreement. I sign it freely and voluntarily.

As Team Contact, I certify that:
  • I have ensured all teammates have read and agreed to the full Waiver and Release
  • I agree to communicate tournament rules, schedules, and updates to my team
  • I acknowledge responsibility for my team's conduct and compliance
